ADOPTION AGREEMENT

The dog is placed as a household pet and to be loved as a member of the family.
The dog is never to be chained or housed outdoors.
Adopted dogs may not be transferred to another owner or otherwise
disposed of, but MUST be returned to the Mid-Atlantic Chinese Shar-Pei Rescue Operation, Inc. (MACSPRO) if no longer wanted or able
to care for said animal.
A representative of the operation may call, inspect or inquire
about the dog before and periodically after it is placed.
All dogs must be spayed or neutered. If, due to health
reasons or age, this has not been done when the dog is received, the applicant
will agree to have the dog spayed or neutered, at owner's expense, and will
have this done as soon as it may safely be done, considering the health
or age of the dog. Veterinary records must be sent to MACSPRO as proof,
and a MASCSPRO representative has the right to physically inspect the dog.
All dogs must have entropion surgery if needed (as determined by a MACSPRO
representative and/or a veterinarian). If you adopt a dog that needs entropion surgery,
or may need it in the future (i.e.; a puppy), and this has not been done when the dog is received
due to health reasons or age, the applicant will agree to have the surgery done, at owner's expense,
and will have this done as soon as it may safely be done, considering the health
or age of the dog.
It is the owner's responsibilty to understand what entropion is, what the indications are, to
watch the dog for signs of entropion, and to contact their veterinarian for confirmation.
Failure to have entropion surgery done when it is needed is not only painful to the dog,
but can utimately lead to blindness.
Veterinary records must be sent to MACSPRO as proof,
and a MACSPRO representative has the right to physically inspect the dog.
The applicant will provide adequate routine veterinary care
including, but not limited to, yearly shots, yearly examination for heart worm,
routine yearly physicals, and year-round heart worm medication. Good health
starts with good care at home; a good diet and healthy, loving home environment.
If the applicant is unable or unwilling to provide needed veterinary care and a
loving, healthy home environment, the dog must
be returned to MACSPRO.
The applicant agrees to accept possession and ownership of
said dog at their own risk and does hereby release MACSPRO from any and all liability arising out
of possession and ownership of said dog. It is understood that MACSPRO
has made no representations concerning the health or condition of the animal
involved and while MACSPRO is willing to accept return of the dog, it is
not to be held responsible for any veterinary expenses incurred while the
dog is not in our possession.
The applicant further agrees to become a member of MACSPRO recognizing the support offered
by MACSPRO. MACSPRO can always be called if you have questions about your Shar-Pei.
It is understood that failure to comply with ANY of the provisions
in this Agreement will result in forfeiture of the adopted dog and reimbursement of legal
expenses incurred to enforce this contract. The dog MUST be returned to MACSPRO.
The applicant is required to keep MACSPRO informed of new address/phone number
changes so we may continue to check on the progress of the dogs we have placed.
By mutual consent, this agreement is pending a pre-adoption home check and
periodic home checks after adoption.
If the applicant should fail to pass the inspections, the dog will be returned
to MACSPRO if corrections can not be agreed upon by parties.
