(MACSPRO)

FOSTER CARE AGREEMENT

1. The dog is placed as a household pet and not for any other purpose. The dog is never to be chained or housed outdoors.


2. The fostered dog may not be transferred to another owner or otherwise disposed of, but MUST be returned to MACSPRO.


3. A representative of MACSPRO may call, inspect or inquire about the dog periodically after it is placed.


4. The applicant will provide adequate routine veterinary care, with the approval and at the expense of MACSPRO, including shots and examination for heart worm. If the applicant is unable or unwilling to provide needed veterinary care, the dog will be returned to MACSPRO or the veterinary clinic they so designate.


5. If the foster care home wishes to adopt the dog, an application must be filed and approved by the rescue committee. It is understand that the foster care home will not automatically be accepted for permanent adoption.


6. 6. The applicant agrees to accept possession of said dog at his or her own risk and does hereby release the Mid-Atlantic Chinese Shar-Pei Rescue Operation, Inc. (MACSPRO) from any and all liability arising out of possession of said dog. It is understood that the Mid-Atlantic Chinese Shar-Pei Rescue Operation, Inc. (MACSPRO) has informed the foster home of the health and condition of the animal to the best of their knowledge. Further, the foster home is requested to speak with the veterinarian who has treated the dog, while in the possession of MACSPRO, for all available information.


7. The dog will be returned to MACSPRO if the foster home can no longer care for the dog or at the request of the rescue committee of MACSPRO.


8. It is understood that failure to comply with the above provisions will result in forfeiture of the adopted dog and/or reimbursement of legal expenses incurred to enforce this contract.

By mutual consent, this agreement is pending a pre-foster home check and periodic home checks thereafter.   If the applicant should fail to pass the inspections, the dog will be returned to MACSPRO if parties cannot agree upon corrections.
